You’re a nurse, a patient advocate and an educator—and you excel at every one of them.The Nurse Navigator will be responsible for the clinical education initiatives to patients and HCP offices for the treatment of Crohn’s Disease/Ulcerative colitis.

Responsibilities
The Nurse Navigator (NN) will play a key role in the education of patients by providing telephonic support and assist in coordination of available resources to patients with Crohn’s disease who have been prescribed a medication. In addition, the NN will conduct educational programs within HCP offices, conduct peer to peer educational programs, and provide patient follow up pre and post infusion and injection training within their assigned territory. The NN will utilize using their professional expertise, relationships and clinical training in the implementation of an education plan.

Hold an active Registered Nurse license in good standing and be licensed in the state in which you are applying for the role.
